
Joseph’s Home Names Anthony Coe Continuity of Care Specialist
Anthony Coe has joined Joseph’s Home, a ministry of the Sisters of Charity Health System, as the continuity of care specialist. In this newly created position, Coe will work with former residents of Joseph’s Home to provide them with ongoing support to aid in their continued recovery and growth.
Coe most recently provided community psychiatric supportive services at Bridgeway, a Cleveland-based provider of mental health services, offering a network of care for those struggling with mental illness and/or substance addictions. He also served in the same role at the Murtis H. Taylor Multi-Service Center in Cleveland, which provides human services, behavioral healthcare and community programs. Prior to that, he served as a community and street outreach worker for Project S.A.F.E., the HIV prevention arm of the Cleveland Treatment Center.
“We are excited to add a continuity of care specialist to the Joseph’s Home staff. With more than 18 years working in counseling, community outreach and social services, Anthony has the skills and experience that are ideal for this new position. He fills an important role in helping us continue to serve our residents even after they leave Joseph’s Home,” says Georgette Jackson, executive director, Joseph’s Home.
In the role as continuity of care specialist, Coe will assess the needs of former residents and assist them in accessing services in the community. He will help them meet ongoing needs, such as medication, food, utility assistance, budget planning and housing. He will also coordinate the delivery of services by Joseph’s Home and other facilities, and develop quarterly social activities for all former residents to attend.
